GOLD Program Nomination Form

Please submit form by Friday, February 21.

  • If you are nominating more than one individual, please submit one form per person and indicate your order of preference.
  • Self-nominations will be accepted
  • Please attach additional documentation as requested below.

Before acceptance, nominees must agree to attend all scheduled sessions in their entirety. ASGE will cover travel and hotel costs to attend the GOLD Retreat in Downers Grove, IL in August 2025. Participants will be expected to attend and fund their own travel and participation at DDW in May 2025 in San Diego, CA.
